- 4대보험가입내역확인서 

* 2025년 10월 1일자로 발급 (날짜 준수)

* 장학금 수혜 여부와 관련 없이 참여 대학원생 모두 제출

* 대표 학생이 취합하여 제출 (대표 학생이 없는 연구실의 경우 선정하여 회신)

We are pleased to provide guidelines on the required documents for students registered as participating graduate students for the Fall Semester 2025.

[Common Documents]

- All participating graduate students must submit the following:

  - Confirmation of Subscription to Four Major Social Insurances (4대보험가입내역확인서)

  * Must be issued on October 1, 2025. (Strict adherence to the date is required.)

  *Must be submitted by all participating graduate students, regardless of scholarship status.

  * Should be collected and submitted by the designated representative student of the lab/group. (If no representative is currently designated, please select one and inform us.)

[New Participating Graduate Students Only]

New participants must submit the following, in addition to the common documents:

- [Attachment 1] Pledge for Participating Personnel (참여인력 서약서)

* Must be signed/dated as of September 1, 2025.

- [Attachment 2] Consent to Collect, Use, and Provide Personal Information (개인정보 수집, 이용, 제공 동의서)

* Must be signed/dated as of September 1, 2025.

- National Researcher Number Inquiry Form (국가연구자번호 조회서)

* Only the Researcher Registration Number may be attached on a sticky note.
